Disease Management Nurse
Summary Description
This position will manage the patient's health status across
the continuum of care. The ideal candidate will implement
strategies to achieve and ensure patient follow-up according
to clinical strategic measures/outcomes. In addition, the
nurse will develop individualized patient/family education
plan focused on self-management; deliver patient/family
education specific to a disease state; coordinate ongoing
education of patients and their families; assist patient and
family to integrate implications of illness recovery and
health promotion into their lifestyle; assist in identifying
patients' educational needs and provide health-related
information to enhance patients' understanding of medical
condition and related treatments; instruct and counsel
patients and families regarding preventive care, medical
problems, psychological problems and use of prescribed
treatments and drugs.
Must Know and Understand
• Internal policies and procedures.
• Have a strong working knowledge of current health
care
protocols.
• Must be able to properly interpret plan documents
for
utilization review, case management, and health care elements.
• Must be familiar with state and federal laws and
resource materials as they pertain to utilization management.
• Have a strong background in patient education.
Responsibilities:
•
Performs on-site and telephonic disease management
assessments and follow-up interventions.
•
Communicates and coordinates on-going health care services
with physicians.
•
Implement strategies to ensure patient follow-up.
•
Develop patient/family educational plan to promote
self-management.
•
Coordinate ongoing educational needs of patients and their
families as related to their disease and self-management.
•
Assist the patient/family with integrating the implications
of the illness recovery and health promotion into their
lifestyle.
•
Instruct and counsel patients/families regarding the disease
and its relation to preventive care, medical problems,
psychological problems and the use of various treatments.
•
Maintains client and subscriber confidentiality at all
times.
Qualifications
• Current RN license in State of Texas
• Certified Case
Manager (preferred)
• Experience with case and disease management
• Organizational skills, self motivated, work independently
• Five years medical/surgical/hospital experience
• Computer skills
• Experience with ICD-9 and/or CPT-4 codes (preferred)
• Willingness to travel up to 30% - Houston, Victoria,
Austin and areas in between
